Our Care Management program at Summit Medical Group is designed to reduce emergency room visits and hospitalizations, improve management of chronic illnesses through interventions and care plans, and reduce length of stay if hospitalized. Our clinical team ensures that the most vulnerable patients receive all necessary services in a supportive and timely manner to achieve optimal health and well-being.

WHAT WE DO

Our care managers engage in advocacy efforts to support and assist patients and their family members throughout the case management process. Patients are given full support in the transition from hospital to home and are linked with providers, medical services, residential, social, behavioral, and community support services.

Care managers conduct in-depth assessments with patients, engaging their family members or significant others in the process, working to:

  • Emphasize prevention, safety and continuity of care
  • Develop a plan that is coordinated and implemented by all involved, including primary and specialty care physicians
  • Monitor and modify care plans, services and outcomes to maximize patients’ health and abilities—all tailored to a patient’s preferences, wishes, and goals

To achieve the goal of maximizing patient functioning, care managers utilize a variety of tools and tactics. In addition to assessing a patient’s understanding of medical conditions and treatment options, care managers work to:

  • Strengthen problem solving and coping capabilities
  • Identify any gaps or barriers that create obstacles to optimal health
  • Enhance ability to interact with and participate in their communities and their medical care
  • Assist in accessing support systems that provide resources and opportunities
  • Counsel patients and families on immediate and long-term care options
