Nancy E. Livingston, MSW, LCSW, CCM, has worked with individuals and families in many different settings along the healthcare continuum, including acute, post-acute, and long-term care. She has extensive experience with the geriatric population and the challenges they face, including adjustment to illness and change in lifestyle due to aging. A fierce advocate for her patients and families, she has expertise in providing support and education, and coordination of resources to empower and assist individuals with whom she works.
Prior to joining Summit Medical Group, Ms. Livingston worked in an acute care hospital assessing the needs of medically complex patients for post-acute care and community social services. She has also worked as a social worker in a long-term care facility as well as served as a liaison for several subacute rehabs. Ms. Livingston has her Case Management Certification from the Commission for Case Manager Certification (CCMC).
When she is not working, Ms. Livingston enjoys running and photography, and loves traveling both inside and outside the country.
