Bruce A. Jordan, M. Div, specializes in providing supportive services for patients with complex medical conditions, including in-home assessments, long-term care planning, caregiver support, end of life counseling, and palliative care.
Mr. Jordan has worked with homeless adults and teenagers, as a hospital chaplain, an associate pastor, and a director of a Salvation Army shelter. Working as a Program Director at Connections, Inc. in Wilmington, Delaware, he led a 13-member Program for Assertive Community Treatment (PACT) team, supporting geriatric adults with mental illness living in the community.
Prior to joining Summit Medical Group, Mr. Jordan was a care manager with the Somerset County Office on Aging, where he helped senior adults to access in-home services to enhance their health and well-being.
He holds a certificate in Palliative Care Chaplaincy from California State University.
When not working with patients, Mr. Jordan enjoys spending time in nature, gaining a greater understanding of history, and taking annual road trips to explore America.
